The Influence of The Arch of The Foot On Knee Injuries

The arch of the foot plays a significant role in maintaining proper lower extremity alignment and can influence the risk of knee injuries. The medial longitudinal arch acts as a natural shock absorber, distributing forces generated during walking, running, and jumping. When the arch functions properly, it helps control foot pronation and supination, allowing forces
to be transmitted efficiently through the ankle, knee, and hip. However, abnormalities in arch height, such as flat feet (pes planus) or high arches (pes cavus), can alter lower limb biomechanics and increase stress on the knee joint.

Individuals with excessively flat feet often experience excessive pronation, causing the tibia to rotate internally. This internal rotation changes the alignment of the knee and may increase stress on the patellofemoral joint, anterior cruciate ligament (ACL), and surrounding soft tissues. As a result, flat-footed individuals may have a greater risk of conditions such as patellofemoral pain syndrome, medial knee pain, and ACL injuries. Conversely, high arches are generally more rigid and absorb less impact during weight-bearing activities. The reduced shock absorption increases the forces transmitted through the knee, potentially contributing to overuse injuries, stress fractures, and degenerative changes.

The relationship between foot arch structure and knee injury highlights the importance of proper biomechanical assessment in athletes and active individuals. Clinicians often evaluate foot posture, gait mechanics, muscle strength, and flexibility when addressing knee pain. Interventions such as strengthening intrinsic foot muscles, improving lower extremity stability,
wearing appropriate footwear, and using custom orthotics when indicated can help optimize foot mechanics and reduce abnormal stresses on the knee. Maintaining healthy arch function is therefore an essential component of injury prevention and long-term knee health.
